Consent

MUST be given for each pupil. Without consent pupils will NOT be
able to take part. This form needs to be kept by you for your records. |

who have been given consent to participate in the Festival. This
form must be completed and returned to Ann Brazier together with
the ticket collation form. |

Ticket
order collation form. The ticket orders from your
school which must be completed and and returned with the correct
money to Ann Brazier together with the consent collation form. |

The
Portsmouth Schools’ Music Festival is at
the Guildhall for three evenings. Wednesday
7th July 2010 ------- Thursday 8th July 2010
------- Friday 9th July 2010 This is
your opportunity to order tickets before the Festival.
Once again, tickets are priced at £4.00, £5.00
and £6.00. If you would
like to pre-order your tickets please follow the steps
below!

make your cheque out for the correct amount payable to the
Portsmouth Schools' Music Festival. Remember to put your
name and address on the back! |
